Dr. Paolo Regi combines a high-volume, solid clinical track record in Hepato-Pancreato-Biliary (HPB) Surgery with continuous and rigorous clinical research activity.
He graduated in Medicine and Surgery cum laude (110/110) from the "Gabriele D’Anunzio" University of Chieti (2005), defending a thesis of major surgical relevance. Subsequently, he obtained his board certification in General Surgery with the highest honors (cum laude, 70/70) in 2010. To internationalize his scientific profile and master Anglo-Saxon methods of clinical data standardization, he completed a Clinical Research Fellowship at the prestigious HPB Unit of the Glasgow Royal Infirmary (Scotland, UK).
On the professional level, he serves as a First-Level Medical Director and Attending Surgeon within the Division of Pancreatic and Hepatic Surgery at the P. Pederzoli Hospital in Peschiera del Garda, a center of excellence and National Reference Hub for pancreatic diseases. He also refined his skills in minimally invasive hepato-biliary oncological surgery at the HPB Surgery Unit of the A.O.U. Mauriziano Umberto I in Turin (Head: Dr. A. Ferrero). To date, his surgical caseload exceeds 2,000 interventions, including over 400 major pancreatic resections performed as lead surgeon, using both open and minimally invasive (laparoscopic and robotic) approaches.
Scientific Activity and Academic Roles
Dr. Regi serves as Scientific Director for specialized training courses and holds teaching positions in Master’s programs and advanced courses in minimally invasive HPB surgery. He is a member of national and international scientific societies, including SICE (Italian Society of Endoscopic Surgery and New Technologies), where he serves as the Coordinator of the HPB Research Group for the 2025-2027 biennium.
His scientific output is documented by peer-reviewed publications indexed in PubMed/Scopus, with contributions appearing in high-impact international journals (such as HPB, European Journal of Surgical Oncology, and Pancreatology). He regularly acts as a peer reviewer for international surgical journals, demonstrating a strong aptitude for the critical appraisal of scientific literature. The decision to pursue a PhD program stems from the need to formalize and expand his research activity in epidemiological, molecular, and biostatistical domains.
RESEARCH STATEMENT — DR. PAOLO REGI
1. Research Lines and Scientific Goals
My research activity stands at the intersection of advanced hepato-pancreato-biliary (HPB) oncological surgery and translational medicine. The primary goal of my scientific program is the optimization of long-term oncological outcomes and the reduction of perioperative morbidity in patients affected by pancreatic ductal adenocarcinoma (PDAC) and cholangiocarcinoma. My research aims to define predictive models and biomarkers capable of guiding personalized surgical selection by integrating clinical findings, immunology, and minimally invasive technologies.
2. Past Scientific Contributions
My research background has focused on the prognostic stratification of bilio-pancreatic tumors and the study of multimodal treatments for advanced stages. I have collaborated on studies aimed at demonstrating the prognostic value of molecular complexes (such as the CXCR4-CXCR7-CXCL12 axis) in post-resection overall survival. Furthermore, I participated in developing biostatistical models to predict surgical margin status (R0/R1) after neoadjuvant therapy, as well as analyzing the systemic immunomodulation effects induced by thermal ablation techniques (RFA).
3. Proposed PhD Research Project
My research activity integrates advanced hepato-pancreato-biliary (HPB) oncological surgery with technology-driven preoperative planning. The objective of this research project is to evaluate the clinical efficacy of segmented 3D models during the preoperative planning phase. Specifically, the primary aim is to assess the diagnostic accuracy of 3D modeling in identifying clinically relevant arterial variants for individual patients, using intraoperative findings as the gold standard. Additionally, the study compares 3D models against standard radiological reports and blind re-evaluations of native images. Finally, the research measures inter- and intra-reader agreement, anatomical classification accuracy (Michels and Hiatt), interpretation times, and reader confidence, aiming to optimize surgical safety and anatomical visualization.
4. Long-Term Academic Goals
My objective is to consolidate my profile as an Academic Surgeon, combining a high-volume surgical practice with the leadership of multicenter clinical trials. The PhD will provide me with the advanced methodological skills in biostatistics and clinical epidemiology required to compete for national and European competitive research grants, thereby fostering the internationalization of the university's research profile.
